How we provide support

Support is framed within an Individual Support Plan which identifies support goals, actions to be taken to achieve these goals, and indicators of success. Support Plans are created in consultation with parents or other primary caregivers, the young person, and other associated service providers. If applicable, an Individual Support Plan will also incorporate goals described in a NDIS Plan. Individual support is general facilitated through a mentoring framework. Cognitive Behaviour Therapy, as ascribed for young people, is available.

Where we provide support

We are located in Albany, Western Australia and our services are delivered within the lower Great Southern region of Western Australia (Albany, Denmark and Mount Barker).

NDIS

Individual Support

Our one-to-one support is specifically tailored to meet the unique needs of children and young adults and is commonly delivered within a mentoring framework. It is through the building of trust relationships that the potential to achieve support goals is maximised. 

Individual support sessions are generally one- or two-hour duration (but can be longer), and depending on the support goals, will occur in an appropriate setting at our premises or out in the community. 

Group Support

Programs are primarily delivered to small groups, however, can be facilitated one-to-one when considered more beneficial. Our group programs primarily focus on social and emotional development, and the building of resilience. The venue for all programs is 12 Sanford Road, Albany. The group programs we currently offer are:

  • PEERS® (Social Skills Development for pre-teens, adolescents and young or older adults)

  • STAR (Successful Transitions and Resilience for 8-11- or 12–16-years of age)

  • COOL KIDS & CHILLED KIDS (Anxiety Management for 8 -11- or 12–16-years of age)

  • The ALERT Program (Self-regulation for 6-9 years of age)

  • EmployABILITY (Development of work readiness skills)
